If I were to rank which shopping mall toilet is the worst to visit, The Curve takes the cake.
I went from the first cubicle to the second then to the third, choosing the cleanest toilet, but to no avail. Most of it has overloaded sanitary bin, unflushed business, and broken toilet seat cover.
Not only the cubicles are in bad condition, the toilet has strong pungent urine smell. There is no toilet roll in each cubicle. You have to get them before you enter the cubicle as soon as you enter the toilet. But most of the time, they ran out.
At least the squat toilet bowl is still usable.

@Lightyoruichi commented that this probably the gayest toilet ever. Hmmm... I'm not really an expert when it comes to male toilet urinals. But it really does not look that male-friendly to me.

Decanter Restaurant, Bukit Damansara
The toilet at Decanter Restaurant, Bukit Damansara is really small. I felt like Gulliver stuck in Lilliput. As I opened the door of the cubicle, I banged my toe. Yes, it is that small! Thank God I didn't die of claustrophobic inside.
The toilet is unisexual. I don't think that is a problem because it's just a small restaurant.
Overall, it is relatively well maintained. The cubicle and basin area was quite clean. There is nothing to complain about except there was no toilet roll! If someone has to do Business No. 2, I wish him/her good luck.

Pavilion Mall, Bukit Bintang
Of all the malls toilet in Malaysia, Pavilion has one of the most satisfying cubicles. The entrance is really spacious. And it has large mirrors for the ladies to check themselves out.
The cubicles are quite clean as the cleaners there are always around to ensure each and every cubicle can accommodate our needs (I'm talking about hygiene here).
But if so happen you're in Parkson, the toilet in Parkson Pavilion is the best! Once you enter the toilet, this is what it looks like:
It looks so warm, almost like a home!
And the cubicle is really spacious that I mistaken it as a disabled toilet.
When I walk towards the end of the toilet, then I realised that the disabled toilet is twice the size of the normal cubicle. I was astonished! The whole surrounding is very clean and most importantly, it is really quiet. You can have the privacy you need to apply your makeups. The lighting there is really good for makeups.
I guess I would walk an extra few steps into Parkson just to enjoy the toilet when I shop in Pavilion the next time.
